New Patient Appointment Preparation

Preparing For Your AppointmentThe physicians and staff at Conejo Pain Specialists Medical Group look forward to meeting with you.  To prepare for your appointment, please review the list of information you need to bring.

  • Please download our Medical History form and complete it before your appointment.  Although some of the questions or information requested may not seem to be relevant to your pain management issue, it is important that you provide as much detail as possible. 
    • If you are not able to download the form, please call our appointment scheduler at 805.497.8616 and a copy will be mailed to you.
  • Please bring prior imaging studies and reports, such as x-rays, CT scans, MRIs.  Also, bring other medical reports, such as nerve or muscle testing results (i.e. EMG).
    • If you do not have these reports, you need to request them from the facility that performed the test.  We have provided a standard Medical Information Release form for your convenience.  Simply complete the form for each provider and make arrangements to either pick up your reports or have them sent to our office.

  • Medical Insurance and Payment for Services
    • We gladly invoice most commercial insurance companies and Medicare on your behalf. 
    • We welcome Workers’ Compensation patients as long as the appropriate referrals and authorizations are provided prior to the appointment.
    • Conejo Pain Specialists Medical Group does not participate in any HMOs, managed care products, Medicaid, or Medi-Cal.
    • On your first visit, please bring your current insurance card and government-issued photo identification (i.e. driver’s license).  Copies of each are placed in your chart.
    • Please advise of changes in your insurance, employment, employer’s medical plan or, if you become eligible for Medicare.
    • Your deductible, copayment, and / or co-insurance are payable when you arrive for your appointment.  Conejo Pain Specialists Medical Group accepts MasterCard, Visa, cash, and checks.

    *During your visit, if additional services are necessary, you will be asked to make additional payment before leaving our office.

    • After your deductible, copayment and /or co-insurance and the insurance company’s payment are applied, you may receive an invoice indicating a balance due.  If you have questions about the balance, or need special payment terms, please contact our billing office at 805.497.8616.

    *In unique situations, your insurance provider may offer assistance if you can document a financial hardship.
